Diabetes Self-Management Program 

diabetes words 2If you are living with diabetes or a caregiver of someone who does, this workshop is for you! Diabetes self-management workshops are interactive learning opportunities that teach techniques to manage common symptoms. Workshops are free and led by trained leaders.

Each participant in the workshop receives a copy of the companion book, Living a Healthy Life with Chronic Condition, and an audio relaxation tape. Classes are highly participatory, where mutual support and success build the participants’ confidence in their ability to manage their health and maintain active and fulfilling lives. Free, but you must register.

Prevent T2

The CDC’s Diabetes Prevention Program helps you take control of your health by adopting habits to reduce your chances of developing type 2 diabetes and improve your overall health and well-being.

The Prevent T2 program provides a supportive environment where participants work together in a small group to learn about healthier eating and increasing their physical activity in order to reduce their risk for developing diabetes. The program, which is led by a trained Lifestyle Coach in a classroom setting, is delivered over a 12-month period, beginning with 16 weekly sessions followed by monthly maintenance. Participants are charged an enrollment fee but upon completion of requirements get reimbursed plus receive a one year free DRC membership. 

In order to qualify for the Diabetes Prevention Program, participants must be at least 18 years old, overweight (BMI ≥ 25) and at high risk for developing type 2 diabetes or have been diagnosed with prediabetes by their physician. Call Debbie at (316) 788-3781 for complete details.

Diabetes Lifestyle Coaching 

Diabetes Lifestyle Coaching is here to assist you in clarifying what you want to achieve, experience, create, do or “be” in order to improve your diabetic health journey. Together you will co-create specific, realistic, measurable goals that are then broken down into smaller, achievable, action steps. The coach facilitates an encouraging, collaborative coaching process to design an individualized step-by-step action plan. The coach and client then set up and utilize accountability and support to create forward movement and progress.

Dining With Diabetes

Nutrition and physical activity are keys to managing type 2 diabetes. Dining with Diabetes is a 4-week program taught by a Registered Dietitian from Sedgwick County Extension. Programs includes the following: 

  • Planning meals & snacks with delicious & healthy recipes
  • Cooking demonstrations & food tastings
  • Motivation & support
  • Ideas for being more active
  • An understanding of how diabetes affects your overall health

Chronic Disease Self Management

If you are living with depression, diabetes, cancer, arthritis, high blood pressure, heart disease, etc., then this 6-week program is for you. This CDSMP workshop is interactive and helps you learn techniques to manage your chronic condition.
